Home & Community Based Waiver (HCB)

  • Target Population: Aged or disabled individuals who meet nursing facility level of care, who without these services may be admitted to a nursing facility.
  • Brief description: HCB waiver is designed to provide intensive services and support to aged or disabled individuals (children & adults) to enable them to remain in or return to their home.
  • Medical & Financial Eligibility: Medical eligible if aged or disabled individuals meet nursing facility level of care. Financial eligibility as determined by local DCBS office based on appropriate income and resource limits.
  • Services Provided: Case Management Services, Minor home adaption, Adult Day Health Care, Homemaker services, Personal Care services, Attendant Care, Respite Care

Supports for Community Living Waiver (SCL)

  • Target Population: Individuals with intellectual and developmental disabilities who meet ICF/MR level of care.
  • Brief Description: SCL waiver is designed to provide intensive services and supports to individuals (children & adults) with intellectual and developmental disabilities to enable them to remain in the community in the least restrictive setting.
  • Medical & Financial Eligibility: Medicaid eligibility if individuals with intellectual and developmental disabilities who meet requirements for residence in an ICF/MR facility. Financial eligibility as determined by local DCBS office based on appropriate income and resource limits.
  • Services Provided: Case Management, Adult Day Training, Behavioral Supports, Children’s Day Habilitation, Psychological services, Residential Support services, Respite services, Supported Employment,

Michelle P. Waiver (MPW)

  • Target Population: Individuals with intellectual or developmental disabilities who meet ICF/MR or nursing facility level of care.
  • Brief Description: MPW waiver is designed to provide services and support to individual (children and adults) with intellectual or developmental disabilities to enable them to remain in the community in the least restrictive setting. Maximum of 40 hours of services per week (alone or in combination).
  • Medical & Financial Eligibility: Medical eligibility if individuals with intellectual (as defined in regulations) or developmental disabilities (as defined in regulations) who meet requirements for residence in an ICF/MR or nursing facility. Financial eligibility as determined by local DCBS office based on appropriate income and resource limits.
  • Services Provided: Case Management, Homemaker services, Personal Care Services, Adult Day Health Care, Adult Day Training, Supported Employment, Community Living Supports, Respite services, Attendant Care, Minor Home Adaptations, Behavioral Supports.

Acquired Brain Injury (ABI)

  • Target Population: Individuals with an acquired brain injury between age 21-65 who meet nursing level of care.
  • Brief Description: ABI rehabilitation waiver is designed to provide intensive services and supports to adults with acquired brain injuries as they work to re-enter community life.
  • Medical & Financial Eligibility: Medical eligibility if individuals have an acquired brain injury between ages 21-65 who meet nursing facility level of care. Financial eligibility as determined by local DCBS office based on appropriate income and resource limits.
  • Services Provided: Case Management, Behavior Programming, Companion Services, Counseling & Training, Group Therapy, Respite care, Structured Day Programs, Supported Employment, Specialized Medical Equipment and supplies, Personal Care, Community residential services.

Model waiver II (MW II)

  • Target Population: Individuals who are dependent on a ventilator at least 12 hours per day and would otherwise require nursing facility level of care in a hospital based nursing facility.
  • Brief Description: Individuals who are ventilator dependent at least 12 hours per day, and would otherwise require nursing facility level of care in a hospital based nursing facility, may receive up to 16 hours per day of registered nurse care or license practical nurse or services of a respiratory therapist.
  • Medical & Financial Eligibility: Medical eligibility individuals who are dependent on a ventilator at least 12 hours per day and would otherwise require nursing facility level of care in a hospital based nursing facility. Financial eligibility as determined by local DCBS office based on appropriate income and resource limits.
  • Services Provided: Maximum of 16 hours per day of registered nurse care or license practical nurse or services of a respiratory therapist.
